Job description

Northeast Colorado Association of Local Governments (NECALG) is seeking a dedicated and compassionate Case Manager to join our Case Management Agency (CMA). This full-time position involves providing professional and technical assessment, case management, and direct service to a diverse member base, including the elderly, blind, disabled, brain injured, mentally ill, developmentally disabled, and those in hospice, pediatric, and home health. The role requires travel within a nine-county area and managing a caseload of 65+ members. We are looking for a detail-oriented team player who is committed to improving the lives of our members.

Responsibilities
  • Attend required meetings, workshops, and seminars.
  • Travel within the nine-county area covered by the agency.
  • Develop care plans and service agreements for members.
  • Monitor the quality of care and ensure the health and safety of members.
  • Act as an advocate for members when appropriate and needed.
  • Comply with all rules and regulations set forth by the State of Colorado.
  • Maintain confidentiality and follow HIPAA regulations.
  • Independently manage and maintain complex records and prepare required reports for the caseload.
  • Deal effectively with conflict and demonstrate strong interpersonal communication skills.
Qualifications
  • Bachelor’s Degree (BA or BS) in a human behavioral science field such as Human Services, Nursing, Social Work, Psychology, or equivalent work experience.
  • Proficiency in writing skills and verbal communication.
  • Knowledge of computers and ability to use programs such as Word and Excel.
  • Client interviewing and assessment skills.
  • Must hold a current driver’s license for the State of Colorado.
  • Must be able to type 60 words per minute.
