Requirements:
Active, unrestricted Colorado RN license
BLS required
BSN required; MSN preferred
Minimum 3 years of hospital clinical experience
Minimum 5 years of hospital case management experience preferred
Case Management certification preferred, or willingness to obtain within 2 years
Experience with utilization management, discharge planning, care coordination, and the continuum of care
Responsibilities:
Coordinate and evaluate patient care plans
Collaborate with physicians, patients, families, payers, and interdisciplinary teams
Ensure appropriate, effective, and cost-efficient utilization of healthcare resources
Perform utilization review and discharge planning
Develop and maintain accurate case management documentation
Coordinate care across the healthcare continuum
Provide education and support to staff and physicians
Collaborate with payers and external review organizations
Support effective transitions of care and appropriate discharge planning
Schedule:
 

Full-Time 36 guaranteed hours per week 5 hours one Saturday per month Receive 5 hours off during the week when weekend coverage is worked Overtime after 40 hours/week No call
