The work setting is entirely remote, allowing you to provide care from the comfort of your home. You will be part of an interdisciplinary team that includes mental health professionals, medical providers, and support staff who collaborate closely to develop integrated treatment plans. Our clinic utilizes advanced telehealth technologies and electronic health records (EHR), ensuring that you have all the resources necessary to effectively assess and manage your patient caseload. The work hours are Monday to Friday, from 9:00 AM to 5:00 PM, providing a balanced work-life schedule.

• Perform comprehensive psychosocial assessments of patients, identifying their individual needs, strengths, and challenges to support effective intervention.
• Deliver exceptional counseling services, health education, and disease management support tailored to each patient's unique circumstances.
• Collaborate with patients and the interdisciplinary healthcare team to design and implement personalized care plans, focusing on behavior modification and coping strategies to enhance mental and emotional well-being.
• Facilitate goal-setting initiatives that empower patients to manage their chronic conditions and related mental health issues effectively.
• Maintain open lines of communication with patients through regular outreach based on their preferences and needs, promoting engagement and positive behavior changes.
• Accurately document all patient interactions, treatment progress, and care plans in the electronic health record (EHR) system in a timely manner to ensure continuity of care.

As an LCSW at Back on Track, you will work with a diverse clientele reflecting various backgrounds, cultures, and life experiences. Our patients may come in seeking help for issues ranging from anxiety and depression to trauma and relationship difficulties. Your role will encompass not only individual therapy but may also involve group sessions and community outreach initiatives designed to educate and support our client's journey toward mental wellness.

• A Master's degree in Social Work from an accredited institution, ensuring a strong theoretical and practical foundation in the field.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania, enabling you to provide direct clinical services.
• A minimum of three years of professional experience in a mental health or outpatient setting, demonstrating a solid understanding of clinical practices.
• Expert knowledge of evidence-based therapeutic modalities and treatment approaches that promote effective patient engagement and success.
• Exceptional interpersonal, communication, assessment, and organizational skills, ensuring effective collaboration with colleagues and patients alike.

• Experience working with a medically underserved population, conveying empathy and understanding to address their unique challenges.
• Proficiency in electronic health record (EHR) systems to efficiently document patient information and treatment plans.
• Demonstrated skills in motivational interviewing and behavioral psychology to facilitate meaningful patient interaction.
