Job description

MINIMUM QUALIFICATIONS

Education – bachelor’s degree is required. A Master of Science in Nursing or Nutrition, a Master of Public Health or a Master of Education is preferred.

Experience – For each level of education stated above, at least two (2) years of experience in the field of diabetes prevention and management or health administration is preferred.

Licenses & Certification –Licensed healthcare professional is preferred. Certification as a Diabetes Educator (CDE) or Certification as a Health Education Specialist (CHES) is preferred.

Knowledge & Skills –

  1. Knowledge of supervisory and managerial principles and skills including program planning and evaluation, directing program activities, funding and staffing in line with program goals.

  2. Knowledge of the principles and practices of health promotion/disease required in developing prevention and management plans.

  3. Knowledge of clinical and nutritional problems associated with diabetes.

  4. Knowledge of the appropriate education and training principles for all age groups as applied to public health practices for instruction of lay and professional groups.

  5. Skill in establishing realistic program goals and objectives, analyzing situations accurately and adopting and implementing effective courses of action.

  6. Ability to communicate courteously and effectively with patients and their families, medical and administrative staff, health care professionals and the general public on an individual and group basis, both orally and in writing.

  7. Ability to utilize a personal computer in a Windows environment.

  8. Ability to maintain a confident professional demeanor.

JOB SUMMARY

The purpose of the position is to coordinate the Diabetes Prevention and Management Program of the Muscogee (Creek) Nation Department of Health (MCNDH). Incumbent plays a vital role in the planning, designing, implementing, monitoring and evaluating of the Program. This is an exempt position as applicable to the FLSA.

WORK ENVIRONMENT

Work is performed in an office setting and in community settings at numerous facilities located within the designated service area. Driving a GSA vehicle is required.

PHYSICAL DEMANDS

Work requires the demands of normal office work, driving to communities and lifting of equipment and teaching aides used in performing position duties.

ESSENTIAL FUNCTIONS

Satisfactory job performance will be determined by successful execution of the following:

  1. Responsible for direction and management of the Diabetes Prevention and Management Program including planning, designing, establishing and evaluating the program; assessing community needs; converting plans to action by setting short-term/long-term objectives and sequencing activities.

  2. Leading progress towards meeting programmatic activities, and monitoring, evaluating and reporting outcomes to the Diabetes Director.
  3. Has oversight responsibility for maintaining the Accreditation of Diabetes Services by updating curriculum and reports outcomes to the Diabetes Director.
  4. Assures the availability of adequate supplies, equipment and facilities; monitors overall status of activities in the work unit identifying problem areas and taking corrective actions.
  5. Assures that the program follows any standards being promulgated by ADA, the IHS and the Muscogee (Creek) Nation.
  6. Assures that all required reports and data are provided to the Indian Health Service (IHS), MCNDH leadership, and facilities/programs in a timely manner and assures that the program is in compliance with any standards being promulgated by ADA, the IHS and the Muscogee (Creek) Nation.

  7. In partnership with the Diabetes Director, oversees and coordinates workgroups and committees in planning activities, services and programs provided through Diabetes Prevention and Management Program, providing guidance, identifying processes and technical expertise in program direction to ensure activities, services and programs are run smoothly, efficiently and meet deadlines or expectations.
  8. Assures broad community and professional input into program activities using formal and informal activities such as Diabetes Advisory Committee, surveys and focus groups located throughout the Muscogee (Creek) Nation.
  9. Collaborates with the Diabetes Program educators and clinical staff in developing curriculum and implementing the diabetes self-management education program.
  10. Collaborates with Diabetes Director to work with Health Administrators and Clinical Directors to promote adherence to standards in diabetes care.

  11. Communicates with MCNDH through meetings and reports.

  12. Monitors licensure of Diabetes Program staff.
  13. Performs other assignments as required.
