What is a TCM?

A TCM (Target Case Manager) job typically involves managing and coordinating services for clients in healthcare, social work, or rehabilitation settings. TCM professionals assess client needs, develop care plans, and connect them with appropriate resources to improve their well-being. They work closely with medical providers, social workers, and community agencies to ensure comprehensive support. Strong organizational and communication skills are essential for success in this role.

What does a TCM do?

A typical day for a TCM practitioner involves meeting with clients to conduct holistic assessments, diagnosing health concerns according to TCM principles, and developing personalized treatment plans that may include acupuncture, herbal remedies, and lifestyle recommendations. You might also spend time preparing herbal formulas, documenting patient progress, and collaborating with other healthcare providers for integrated care. Administrative tasks like scheduling, maintaining records, and staying updated with continuing education are also common. This role offers a dynamic mix of direct patient care and ongoing professional development in natural healthcare settings.

What are the key skills and qualifications needed to thrive in the TCM position, and why are they important?

To thrive as a TCM (Traditional Chinese Medicine) practitioner, you typically need formal education in TCM, licensure or certification as required by your region, and a deep understanding of herbal medicine, acupuncture, and diagnostic techniques. Familiarity with electronic health records (EHR), herbal prescription software, and acupuncture tools is also important. Excellent communication skills, empathy, and cultural sensitivity help build strong relationships with clients and colleagues. These capabilities are essential for delivering safe, personalized, and effective holistic healthcare to a diverse patient population.

The City Mission

Full-time

This job post has expired today. Applications are no longer accepted.

Job description

We are now accepting Unpaid Social Work Practicum applications for Fall 2026.

About us:

As a Christ centered organization, we are dedicated to reaching hearts and changing lives by caring for and empowering individuals experiencing homelessness in Cleveland. All interns at The City Mission (TCM) are ministers of the Gospel and are vital for the fulfillment of our mission: providing help and hope to all people through the transforming power of God's love. Therefore, it is essential that all staff, interns, and practicum students of TCM have a personal relationship with Jesus Christ and agree with and abide by our Statement of Faith and Lifestyle Principles.

These core values represent our staff and interns/practicum students, and we will recruit, reward, recognize, review, coach, and correct based on these core values:

  • We are called to serve Christ and others
  • We believe God transforms lives
  • We commit to steward well
  • We succeed by working together
  • We care for every person knowing all are made in the image of God

Unpaid Practicum Summary:

The City Mission offers unpaid Social Work practicums in the fall and spring semesters. The requirements of your school will determine the social work practicum experience and who will be able to supervisor the practicum. Anyone pursuing an unpaid practicum with The City Mission must be doing so for school credit.

Please upload your resume, letter of interest, and school requirements to be considered. HR will then be able to determine if there are available opportunities within TCM based on the school requirements.
