Your Role: Home Health Speech Language Pathologist

You will provide one-on-one care in patients' homes, helping patients improve communication, cognitive skills, swallowing function, and confidence in daily living.


Key Responsibilities
  • Conduct in-home speech therapy evaluations and follow-up visits
  • Develop and implement individualized treatment plans
  • Assess and treat speech, language, cognitive, voice, and swallowing disorders
  • Educate patients and caregivers on communication techniques and home exercises
  • Document visits using WellSky Kinnser EMR
  • Collaborate with physicians, RNs, and the interdisciplinary care team

Qualifications
  • Active Arizona Speech Language Pathologist license
  • Master's Degree in Speech-Language Pathology or related accredited program
  • ASHA Certificate of Clinical Competence (CCC-SLP) preferred
  • CFY candidates welcome to apply
  • CPR certification (AHA) preferred
  • Minimum 1 year of clinical experience (home health preferred)
  • Strong clinical judgment and time management
  • Reliable transportation, valid driver's license, and insurance
  • Strong communication and a patient-first approach
