Job description

Help people build healthier lives through compassionate, person-centered nutrition care that makes a meaningful difference every day. Full-time, benefit eligible position. The schedule is Monday - Friday 8:00am-5:00pm. As a Registered Dietitian, you'll be an integral member of our integrated care team, providing nutrition assessment, education, and counseling to individuals enrolled in our Healthcare Home program. You'll partner with behavioral health and primary care professionals to support whole-person wellness, empower healthy lifestyle changes, and develop practical nutrition strategies that meet each person's unique needs and goals. Key Responsibilities Provide individualized nutrition assessment, counseling, and education for clients and their families in both office and community settings. Collaborate with integrated care team members to support coordinated, person-centered care. Serve as a nutrition resource by providing consultation to staff and participating in interdisciplinary case discussions. Develop and deliver nutrition education for clients, families, and staff through individual and group presentations. Partner with Healthcare Home leadership to develop, implement, and enhance nutrition-related care pathways and wellness initiatives. Support health behavior change using motivational interviewing and other evidence-based approaches. Serve as the Registered Dietitian and nutrition consultant for Healthcare Home enrollees. Maintain CPR certification and perform CPR when necessary in accordance with organizational protocols. Perform other duties as assigned. Requirements, Skills, Knowledge and Expertise Bachelor’s degree required Must have dietary knowledge as it specifically relates to the diagnoses of Healthcare Home enrollees (i.e. chronic medical conditions and various behavioral health conditions in adults and children) Must have a working knowledge of budget conscious healthy eating plans (i.e. for people who have a limited food budget, those that depend on food stamps, food pantries, etc.) Must have a working knowledge of techniques for affecting health behavioral change for clients with a Serious Mental Illness Licensure/Certification Registered Dietitian Nutritionist (RDN) by the Commission on Dietetic Registration (CDR) Must obtain and maintain current CPR certification within 60 days of hire You'll be a great fit for this role if you: Enjoy helping people achieve healthier lifestyles through practical, individualized nutrition guidance.
