Job description

Non-Certified Support Coordinator

Summary: Monitor service delivery of caseload

Position Type: Full Time, Non-Exempt

Essential Functions:

  • Review referrals and assess program eligibility
  • Complete intake and initial assessment of services timely
  • Adhere to all of the OAAS policies, procedures and timelines
  • Develop and monitor service plans in accordance with regulatory guidelines
  • Maintain contact with program participants in accordance with regulatory guidelines
  • Assist with developing and monitoring monthly budgets, where applicable, for program

participants

  • Assist program participants with obtaining community support, resources and services
  • Maintain required documentation (service logs, POCs, etc)
  • Submit service logs and billing documentation according to procedure
  • Attend all OAAS Mandatory meetings and trainings
  • Check and respond to all phone calls daily
  • Maintain training requirements
  • Report any significant concerns and critical incidents to supervisor/SIMS
  • Rotate on call 24 hour support to program participants

Required Education and Experience: Bachelor’s degree in the Human Service Field. Within first three months of employment, must become certified in IHC and Plan of Care. Maintain valid driver’s license and vehicle insurance, excellent writing and computer skills required.

The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
