Atria Health Institute Integrative Medicine Advanced Care Practitioner

The Atria Health Institute is a membership-based primary and specialty health care practice with a focus on prevention and longevity. We bring together a multidisciplinary team of renowned physicians to provide proactive, preventive, and precision-based care for Atria members and their families. All care, including primary care, advanced screening and diagnostics, urgent care, specialty care, 24/7 home visits, and imaging is included in members' annual fee.

Our mission is to make healthspan and lifespan equal for all by translating science into medicine in real-time, all while bringing humanity back into health care. Delivering such robust, personalized, and preventive health care is complex and requires a team-wide dedication to excellence.

We successfully opened our flagship Institute in New York in 2022, expanded to South Florida in 2024, and will be bringing the Atria experience to the West Coast with the launch of our Los Angeles Institute in summer 2026.

This is an opportunity to support the Integrative Medicine team at Atria, with a primary focus on Lifestyle Medicine as an early leader in our Precision Lifestyle Medicine Initiative. The Lifestyle Medicine-focused Advanced Care Practitioner (NP or PA) in this role will oversee all lifestyle-related care for a cohort of Atria members in partnership with each member's Chief Medical Officer and Atria's Nutrition, Performance and Movement, Sleep Medicine, Mind-Body, Behavioral Medicine, and Environmental Medicine experts. You will be working with a member population that has chosen the highest level of personalized, proactive care available. The ideal candidate brings a strong internal medicine or primary care foundation, genuine fluency in functional and integrative medicine principles, and the clinical curiosity to continually deepen their expertise alongside a team of physicians who are themselves defining the frontier of this field.

This role is highly collaborative and member-facing. The ideal candidate is organized, communicative, observant, and comfortable working within teams, ideally with prior experience working in Integrative / Holistic Medicine settings. You are proactive, detail-oriented, and skilled at building accountability and follow-through with members.

What You'll Do:

  • In partnership with Atria's Integrative Medicine team, conduct thorough patient evaluations — gathering detailed medical histories, performing physical examinations, and synthesizing diagnostic findings to develop a complete picture of each member's health
  • Diagnose and manage acute and chronic conditions within the integrative medicine scope of practice, with particular emphasis on Lifestyle Medicine, Environmental Medicine, and Mind-body Medicine as core determinants of health
  • In partnership with Atria's Integrative Medicine team, ordering relevant diagnostic testing on a case-specific basis
  • Develop and implement individualized treatment plans in collaboration with the member's Primacy Care physician and related specialists
  • Monitor member progress, adapt treatment plans in response to evolving clinical findings, and maintain proactive, longitudinal communication with members about their health trajectory
  • Perform Remote Patient Monitoring in the areas of sleep, nutrition, performance, movement, and environment
  • Maintain accurate, thorough, and timely clinical documentation in Athena — upholding the highest standards of medical record integrity and regulatory compliance
  • Support the development of Atria's novel Precision Lifestyle Medicine Program, with focus on member facing care plan unification among Atria's various lifestyle specialties, behavior change education and encouragement, and proactive member outreach
  • Support the development of Atria's novel Micronutrient Optimization Program, with focus on member facing care plan development
  • Function as an integral member of the Integrative Medicine clinical team — contributing to team meetings, case discussions, and collaborative treatment planning
  • Coordinate care across disciplines with intentionality — facilitating referrals grounded in clinical reasoning, ensuring continuity across every touchpoint in the member's journey
  • Partner the care team on clinical workflow design, member communication protocols, and the operational systems that support seamless, high-quality care delivery
  • Stay at the forefront of Integrative and Lifestyle Medicine — engaging in continuing education, professional organizations, and relevant conferences to ensure clinical practice reflects the most current evidence
  • Bring intellectual curiosity and a spirit of inquiry to the team — contributing to research initiatives, clinical framework development, and the collective pursuit of clinical excellence

Compensation: $160,000 - $190,000/year, depending on years of experience
