You will be working closely with an interdisciplinary team of healthcare providers, including physical therapists, nurses, and social workers, who share a common goal of supporting resident recovery and promoting the highest standards of care.

• Conduct comprehensive assessments of residents’ physical, emotional, and cognitive needs to create customized treatment plans.
• Implement one-on-one therapeutic interventions aimed at improving residents’ mobility, daily living skills, and overall quality of life.
• Continuously monitor patient progress, adjusting therapy plans as necessary while maintaining thorough documentation in our electronic medical records (EMR) system.
• Collaborate with interdisciplinary teams to ensure coordinated care that aligns with the individualized needs of each resident.
• Educate residents and their families on techniques and exercises that promote independence and safety in daily activities.
• Adhere to healthcare regulations and best practices to ensure the delivery of safe and effective occupational therapy services.

As an Occupational Therapist at Forward Function Healthcare Center, you will work primarily with seniors and individuals recovering from surgeries, injuries, or illnesses that impact their day-to-day functioning. Our client population includes those within assisted living arrangements who need specialized support to regain their independence and improve their quality of life. Understanding the unique needs and challenges faced by our diverse residents is essential in delivering impactful therapy solutions.

• A Bachelor's degree in Occupational Therapy from an accredited program.
• An active Occupational Therapist license in the state of Oregon is required.
• A minimum of two years of experience in a healthcare setting, preferably within rehabilitation or geriatric care.
• Proficiency in utilizing EMR systems and familiarity with current occupational therapy techniques and industry regulations.

• Experience working in a rehabilitation center or assisted living environment.
• Specialization or certification in geriatrics or related fields.
• Demonstrated ability to develop and implement engaging therapy programs focused on improving patient outcomes.

The Occupational Therapist position at Forward Function Healthcare Center offers a competitive salary range of USD 90,000 to 100,000 annually.
