Care Coordinator - Heritage Behavioral Health Center | Decatur, Illinois
Position: On-Site at 151 North Main, Decatur, IL Full-Time | Monday-Friday, 8 AM-5 PM | Every Other Friday Off (Paid Wellness Day)
Pay:

  • High School Diploma or equivalent + 5 years of supervised mental health or social service experience: $23.00/hour
  • Bachelor's Degree in Human Services, Psychology, Social Work, or Education: $25.00/hour

WHAT YOU'LL DO
As a Care Coordinator, you'll be the connective thread that keeps clients engaged in their care from first contact through successful treatment. You will:
  • Ensure clients get connected to behavioral health treatment, primary care, specialty care, and community resources
  • Follow up proactively with clients to track progress and troubleshoot barriers to care
  • Coordinate with primary care teams, behavioral health providers, and community partners on appointments and treatment plans
  • Educate clients and families on care continuity and available resources
  • Document client progress and service engagement in electronic health records
  • Help clients navigate healthcare and social service systems
  • Participate in interdisciplinary team meetings and ongoing trainings
WHAT YOU BRING
  • High School Diploma/GED plus 5 years of supervised mental health or social service experience, OR a Bachelor's degree in social work, psychology, education, or a related human services field
  • Minimum age of 21
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • Strong communication skills and the ability to build rapport with diverse clients
  • Comfort with electronic health records (EHR) and Microsoft Office
  • A collaborative, adaptable, and empathetic approach to client care
