Job description

Job Title

Perform tasks as specified by Director of Care Transitions leadership team.

Maintains an orderly flow of patients.

Follows OSHA standards and Infection Control policies.

Seeks direction and supervision, when necessary, but functions independently where appropriate.

Correctly identifies all patients by dual identifier prior to performing any procedures.

Demonstrates flexibility with changing assignments due to staffing needs and adequately manages workload.

Performs related clerical duties to ensure continuity of patient care.

Utilizes computer technology in the work area.

Enter data accurately.

Discharges patients and make referrals as indicated by the team.

Maintains a safe environment for patients and report unsafe condition to the unit manager.

Utilizes communication technique effectively.

Demonstrates a professional caring, respectful approach with patients, families and co-workers.

Understands and respects cultural differences.

Communicates in a pleasant, courteous, respectful manner always.

Maintains confidentiality and privileged communication.

Uses effective communication to assist in directing patient flow.

Assumes responsibilities in knowing St. Barnabas' Policy and Procedures and Standard of Care.

Age Specific Competencies

Adult and Geriatric Population

Modifies approaches based on age-specific needs and responses to treatment.

Uses communication techniques that are age appropriate.

Specifics Navigators' Responsibilities-

Completed readmission check sheet in a timely manner.

Provide patients with adequate resources such as housing, food, and legal services.

Referral to community agencies.

Re-access patients that are re-admitted to a medical unit within 30 days of last discharge for COPD and/ or CHF.

Assess patients in the ED with COPD and/or CHF to avoid admission if possible

EMR knowledge and skills – EPIC

Documents patient care navigator notes.

Email/telephone knowledge and etiquette

Respond to incoming emails and calls in a timely matter.

Ensure out of office notifications are activated on scheduled days off.

Review emails and voicemails on a daily basis.

Education Required –

Associate degree at minimum

Experience/Skills Required

  • Previous experience in a medical setting or 2 years relevant work experience preferred
  • Excellent communication/ interpersonal skills
  • Bilingual preferred
  • Knowledge of computer technology preferred. Successful completion of St. Barnabas Hospital orientation.

Working Conditions:

Thirty–five (35) hours per week (7.0) per day as assigned. Demanding and fast-paced office environment with direct patient contact. Moderate heavy physical activity including considerable walking, pushing, pulling and lifting.
