Job Summary:

At RegalCare at Lowell, our commitment to providing exceptional care and support to our patients and residents is paramount. As a Social Worker, you will play a vital role in promoting their overall well-being and quality of life. This part-time position offers a unique opportunity to make a meaningful impact in the lives of others.

Responsibilities:

  • Conduct initial and ongoing assessments of patients' and residents' physical, emotional, and social needs

  • Develop and implement personalized care plans to address their needs and goals

  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery

  • Provide counseling and support to patients and residents, as well as their families and caregivers

  • Facilitate communication among patients, families, and healthcare providers

  • Identify and address social determinants of health that impact patients' and residents' well-being

Requirements:

  • Licensed Clinical Social Worker (LCSW) in the state of Massachusetts

  • Previous experience in Long-Term Care (LTC) is preferred

  • Strong communication, interpersonal, and problem-solving skills

  • Ability to work independently and as part of a team

  • Compassionate and empathetic approach to patient care
