The Marvin and Betty Danto Health Care Center is seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. Located in Oakland County, MI, our facility is committed to providing exceptional care and services to our residents and patients.

About the Role:

Social Worker

In this full-time position, you will work closely with our interdisciplinary team to provide social work services to our patients, including assessments, counseling, and planning. You will identify and address the psychosocial needs of our patients, including those related to mental health, cognitive impairment, and end-of-life care. Your expertise will be invaluable in helping our patients and families navigate the complexities of healthcare and rehabilitation.

Responsibilities:

• Conduct comprehensive social work assessments and evaluations to identify patients' needs and develop individualized care plans
• Provide counseling services to patients and their families to address concerns and promote emotional well-being
• Facilitate patient and family education and support groups to address specific needs and promote healthy coping mechanisms
• Collaborate with our interdisciplinary team to develop and implement care plans that address patients' psychosocial needs
• Identify and address social and environmental barriers that impact patients' health and well-being
• Maintain accurate and timely records of patient interactions and care plans
