Position Summary

We are seeking a compassionate and detail-oriented Social Work Assistant / Social Services Designee to support the psychosocial well-being of residents in our skilled nursing facility.

This role offers a flexible 3-day-per-week schedule, allowing you to select the days that work best for you.

You will assess resident needs, coordinate services, and support care planning while ensuring compliance with CMS regulations, MDS, and NASW standards.

Key Responsibilities
  • Conduct psychosocial assessments and complete social histories

  • Participate in MDS assessments, care plans, and interdisciplinary meetings

  • Provide emotional support and adjustment counseling for residents and families

  • Coordinate discharge planning and community resource referrals

  • Maintain accurate documentation (MDS, care plans, progress notes, PASARR, etc.)

  • Support resident rights, advocacy, and quality of life initiatives

  • Assist with survey readiness and compliance (CMS/state regulations)

  • Investigate concerns and support abuse prevention and reporting protocols

  • Collaborate with nursing, therapy, and administration teams

Qualifications
  • Licensed or certified in Social Work or Human Services (state requirements apply)

  • Bachelorโ€™s degree in Social Work, Human Services, Psychology, or related field

  • 1+ year of social work experience in healthcare or long-term care

  • Skilled nursing facility (SNF/LTC) experience preferred

  • Knowledge of MDS, care planning, and CMS regulations strongly preferred

  • Strong communication, organization, and documentation skills
