) Fairfax Rehab & Nursing Center is seeking a Social Work & Discharge Planning Assistant. The ideal candidate must be devoted and compassionate about their chosen career and ensure that the social, psychological, and physical needs of all residents in the center are being met. We are looking for a Social Work & Discharge Planning Assistant who is dependable, self-motivated, a team player, and flexible. Social Workers are the advocates for our residents and play a very important role.
Qualifications for Social Work & Discharge Planning Assistant:
  • Bachelor's degree in social work, human services, or related field preferred.
  • Two (2) years of experience in Social Work or related role.
  • Experience in skilled nursing or healthcare environment preferred.

Skills of Social Work & Discharge Planning Assistant. All qualified applicants should possess the following qualities:
  • Strong interpersonal skills and ability to communicate effectively.
  • Strong organizational and analytical skills.
  • Strong written and verbal communication skills.
  • Ability to work independently with limited supervision.
