Job Summary:

As a Designated Coordinator, you will oversee the program management for homes supporting individuals with disabilities. In this role, you will assure each individual is provided guidance, encouragement, support, comfort, security, and safety through relationship building. You will advocate that each individual's health and wellness needs are met and that they are accomplishing their outcomes, hopes, and dreams by promoting independence in performing essential life skills. You will monitor and assure the administration of medication and medical treatments as prescribed by physicians. You will monitor and maintain each individual's plan and financial records per regulation and program policies and procedures.

In this role, you will also supervise, support, train, and evaluate Direct Support Professionals - teaching them to be the advocates they need to be to support each individual they work with. You will be responsible for accounts payable, payroll records, petty cash/individual accounts, and budget monitoring. You will develop and maintain positive working relationships with counties, families, stakeholders, and community organizations/groups. You will be required to maintain documentation, perform record keeping, and participate in periodic meetings/trainings. Although the hours for this position are primarily Monday-Friday, the expectation is that you will be available as needed for emergencies and to meet the needs of the program.

Qualifications and Requirements:

  • A bachelor's degree in a field related to Human Services and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
  • An associate's degree in a field related to Human Services and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
  • A diploma in a field related to Human Services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
  • A minimum of 50 hours of education and training related to Human Services and disabilities and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses 1 to 3.
  • When managing an ICF/MR program, a bachelor's degree (with QDDP designation) is required.
  • Driver's license required.
  • CPR certificate, First Aid (when required), and medication administration training is also required but can be obtained after hire.
  • Training in supervisory skills, team building, and leadership.
  • Bureau of Criminal Affairs (BCA) clearance is required.
  • Motor Vehicle Check (MVR) with a satisfactory driving record is required.

  • 40% FTE or greater

  • Maintain employment in good standing
  • Work all scheduled shifts
