Holistic Harmony Associates is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time position focused on providing critical support to clients within a hospice center setting. Work Environment

This position is fully remote, allowing for flexibility while maintaining a collaborative approach with our team and community partners. Key Responsibilities
  • Conduct thorough assessments of clients’ social, emotional, and spiritual needs, guiding comprehensive care for patients in hospice.
  • Develop individualized care plans that incorporate personalized goals, interventions, and strategies for progress monitoring.
  • Provide empathetic counseling and therapeutic support to individuals and families facing complex life transitions and grief.
  • Engage in crisis intervention, offering immediate risk assessment and safety planning for clients in vulnerable situations.

  • Participate actively in community outreach activities to raise awareness about hospice services and the importance of holistic care.
  • Document all client interactions and care plans within our electronic health record system, ensuring compliance with agency standards.
  • Contribute to team discussions and training sessions aimed at enhancing service delivery and professional growth.
Patient Population / Client Focus

In this role, you will primarily focus on clients in hospice care who are navigating complex emotions and situations associated with terminal illness, including their families. Your efforts will directly contribute to enhancing the quality of life for these clients by ensuring they receive compassionate care, emotional support, and vital resources during their time of need.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • An active state license as a Licensed Clinical Social Worker (LCSW).
  • Solid understanding of hospice care principles, end-of-life issues, and grief counseling.
  • Exceptional communication and interpersonal skills, allowing for effective collaboration with clients and colleagues from diverse backgrounds.
  • Strong analytical and problem-solving abilities to address clients’ challenges efficiently.
Preferred Experience
