Job Title: Social Services Tech

Shift: Full-Time (40 hours per week on average)

Job Hours: Times May Vary

Base Location: Gwinnett/Rockdale/Newton/Clayton Counties

Division/Department/Program: Intellectual Developmental Disabilities (IDD)Residential

The IDD Site Supervisor manages operations, supervises direct care staff, and monitors resident activity in a community residential site serving consumers with Intellectual/Developmental Disabilities.

Supervises, guides, and/or instructs the work assignments of subordinate staff. Assists with daily living activities at the person's home, in a daytime non-residential facility or a residential facility. Observes and monitors client's behavior and charts observations and incidents. Provides training of basic personal and social skills, simple academics, or work skills to consumers/students in a state school, hospital, juvenile detention facility, residence, group home, workshop, or supported employment environment. May assist with meal preparation, ensuring or encouraging client group activity, providing transportation, and/or limited case management.

Duties & Responsibilities:

  • Provide crisis intervention by responding ot and managing emergency situations in the Residential home on a 24-hour basis, reporting emergencies to the IDD manager/director within one business day.
  • Ensure communication sheets are completed and submitted within 24-hours of the incident.
  • Ensure adequate Residential Care coverage by creating and posting a monthly staff schedule in the home, compliant with HFR standards (Currently collaborating with IDD director).
  • Verify the Residential Care Providers maintain all required trainings and certifications (e.g. CPR, First Aid, Relias) without lapses, and complete 16 hours of personal training annually as required (currently collaborating with IDD manager/director.
  • Ensure compliance with Healthcare Facility Regulations (HFR) guidelines, maintaining clean and organized bedrooms, bathrooms, floors, and cabinets, with regular vacuuming completed.
  • Monitor and maintain sufficient food, supplies, and emergency food stocks.
  • Ensure residents are safe, healthy, and receive adequate Individual Support Plan (ISP) training, with all personal supplies provided as needed.
  • Train and monitor staff to implement Person-Centered Supports, ensuring adherence to residents' ISP's.
  • Observe and accurately document resident progress based on interviews or interactions, writing concise, clear observation notes in the Electronic health Record (EHR) per csB policy.

  • Promptly notify appropriate professionals (e.g. IDD Nurse Supervisor) of any unusual incidents or situations.
  • Ensure accurate completion of Medication Administration Records (MARs) and medication records by RCPs.
  • Coordinate annual physicals, dental exams every six months, prompt follow-ups on medical issues, and ensure Behavioral Support Plans (BSPs), if needed, are in place with proper documentation.
  • Monitor and document any restrictions on resident rights, ensuring compliance with DBHDD standards.
  • Submit maintenance requests within 24-hours and follow up to ensure timely resolution.
  • Facilitate a variety of affordable and meaningful community and home-based activities to engage residents, aligning with their ISPs.
  • Other related duties as assigned.

Minimum Qualifications:

High School diploma/GED and three (3) years of job-related experience providing social or direct care services to groups of special needs individuals in a human service setting, which includes one (1) year experience as a lead worker, team lead, or supervisory role; or One (1) year of experience at the lower-level Client Support Worker 3 (SSS012) or equivalent position.

Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Any combination of training and experience which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.

Note: Some positions may require a valid driver's license.
