What You'll Do
  • Complete weekend Start of Care (SOC) admissions in patients' homes.
  • Perform comprehensive nursing assessments and complete OASIS documentation.
  • Develop and initiate individualized plans of care.
  • Identify clinical, medication, safety, and disease-management needs.
  • Educate patients and caregivers on medications, diagnoses, symptoms, and home care.
  • Coordinate necessary services with nurses, therapists, physicians, and other members of the interdisciplinary team.
  • Complete accurate and timely electronic documentation.
What We're Looking For
  • Active Minnesota Registered Nurse (RN) license.
  • Current CPR certification.
  • Previous home health experience required.
  • Strong working knowledge of OASIS and Start of Care admissions.
  • Excellent clinical assessment and documentation skills.
  • Confidence working independently in patients' homes.
  • Valid driver's license, active auto insurance, and reliable transportation.
