Position Summary

We are seeking a skilled and caring Registered Nurse (RN) to join our Home Health team. You will provide in-home skilled nursing visits, patient assessments, education, and care coordination. This role requires strong clinical judgment, autonomy, and excellent communication skills.
Key Responsibilities

  • Complete in-home skilled nursing assessments and treatments
  • Administer medications and perform wound care, injections, and other skilled procedures
  • Collaborate with interdisciplinary team (MD, PT, OT, MSW, HHA)
  • Update and follow care plans per physician orders
  • Provide health education to patients and caregivers
  • Maintain accurate and timely documentation (EHR)
  • Communicate changes in condition promptly
  • Travel locally to patient homes in Covina, Glendora, Walnut areas
Qualifications

  • Active California RN license (required)
  • Home Health experience preferred but not required
  • Strong communication and clinical assessment skills
  • Reliable transportation and valid drivers license
  • Ability to work independently
  • Knowledge of OASIS (preferred but can omit if you want)
