As a Clinical Success Mentor, you'll contribute to our success in the following ways:

  • Act as a liaison and maintain communication with new hire and current clinicians, branch leadership, and preceptors to provide practical clinical experience and guidance to field clinicians, including standardized orientation of new clinicians.
  • Ensure execution of the new hire field clinician orientation experience via:
  • Preceptor Guide
  • Competency Checklist
  • State specific requirements as needed
  • Demonstrate processes for SOC, ROC, Recert and routine visits to clinicians during orientation process
  • Work with preceptors and/or clinicians to review application of clinical documentation that meets regulatory and compliance standards and guidelines
  • Provide education to ensure effective collaboration with healthcare providers in accordance with physician's orders and plan of care.
  • Continually work to create a culture of learning and support for preceptors and new clinicians as they are gaining confidence and competence in their respective role.
  • Meets with Preceptors and/or Orientees on regular basis to assess progress during Orientation. Communicates with CTMs and/or BDs as needed to discuss progress of Orientees.
  • Collaborate with preceptors and local leadership team to determine & participate in the delivery of targeted education based on needs and process improvements to align with Elara goals and vision
  • Facilitate annual skills lab & competency assessments as needed
  • Other duties as assigned to support the changing needs of the business.

What is Required?

  • Diploma, Associae or Baccalaureate Degree in nursing
  • 1+ year home health experience
  • Previous nurse preceptor or educator experience is preferred
  • Must have a current, unrestricted RN license in the state you are working, may be required to obtain additional state licensure.
  • Proficiency with HomeCare HomeBase & PointCare is preferred.
  • Demonstrates strong leadership, effective communication and interpersonal skills.
  • Ability to travel 50% travel required (mainly to branches in your region)
  • Proficiency with OASIS

You will report to the Director of Education.
