We are currently seeking an experienced and reliable Registered Nurse (RN) to join our skilled home health team.

Position Overview

The Home Health RN will provide skilled nursing visits to patients in their homes throughout the DFW area. This role requires strong clinical skills, independence, and excellent documentation abilities.

Responsibilities

• Perform Start of Care (SOC), Recertifications, and Routine Skilled Nursing Visits
• Complete accurate and timely documentation
• Assess patient condition and develop care plans
• Administer medications and treatments as ordered
• Educate patients and families on disease management
• Communicate effectively with physicians and care team
• Travel to patient homes within assigned territory

Qualifications

• Active Texas RNlicense (Required)
• Minimum 1 year nursing experience (Home Health preferred)
• Strong assessment and critical thinking skills
• Reliable transportation and valid driver’s license
• Ability to work independently
• OASIS experience preferred
