Job Description
Territory coverage includes both Central DuPage Hospital and Palos Heights Hospital regions.
Responsible for the development, implementation, and maintenance of the Wound, Ostomy, Continence Program of Palos Community Hospital Home Health. Assesses and treats or makes recommendations for treatment of the physical and psychosocial status of patients with ostomies, incontinence, multi/complex wounds, pressure sores, and related skin problems. Provides direct care to patients under the RN job description.
RESPONSIBILITIES:
  • Assesses patient needs with specific attention to ostomies, multi/complex wounds, pressure sores, and incontinence-related skin problems.
  • Provides direct patient care under the RN job description with specific duties and responsibilities related to ostomies, multi/complex wounds, pressure sores, and incontinence-related skin problems.
  • Assesses needs and recommends appropriate products, devices, and/or supplies to achieve positive outcomes in a cost-effective manner.
  • Identifies appropriate nursing interventions; instructs, oversees, and guides implementation of the plan of care to nursing staff.
  • Acts as the primary resource for referral and care of patients with ostomies, complex wounds, pressure sores, and incontinence-related skin problems.
  • Prepares and maintains written reference materials for patient and staff education.
  • Assesses staff education needs and provides in-service programs as indicated.
  • Maintains awareness of and updates staff regarding recent changes and recommendations in the management of wound, ostomy, and incontinence problems.
  • Maintains competency through involvement with professional organizations and attendance at relevant educational offerings.
  • Participates in development, implementation, and review of policies and procedures related to ostomy, multi/complex wound, pressure sore, and incontinence-related skin care; recommends changes when necessary.
  • Participates in Quality Improvement (QI) activities, including data collection, analysis, and recommendations related to the scope of care provided.
  • Documents patient care in accordance with the RN job description and agency policies and procedures.
  • Demonstrates ability to care for patients with understanding of cognitive, physical, emotional, and chronological maturation processes.
  • Adheres to PCH Home Health policies regarding HIPAA privacy and confidentiality rules, dress code, and attendance.
  • Adheres to all PCH and PCH Home Health policies.
  • Performs other duties and responsibilities as indicated in the RN job description.
  • Performs other related duties as directed.
Qualifications
Required:
  • Bachelor of Science in Nursing (BSN)
  • Current State of Illinois Registered Professional Nurse
  • American Heart Association Basic Life Support (BLS)
  • Evidence of satisfactory completion of an approved course of instruction for Enterostomal or Wound, Ostomy, Continence nursing
  • Strong interpersonal skills
  • Ability to collaborate with others
  • Strong analytic abilities
  • Maintains active driver's license and auto insurance
  • Associate Degree in Nursing (ADN) acceptable with commitment to obtain a BSN from an accredited program within three years of first day of employment in clinical nurse role.
Preferred:
  • 2 years' experience as an Enterstomal/Wound, Ostomy, Continence Nurse preferred
  • Certification as an Enterostomal/Wound, Ostomy, Continence RN preferred
  • Previous home care experience preferred
