Work Environment

This position is onsite and will require traveling to patientsโ€™ homes within Camden and surrounding areas. In this role, you will work closely with patients in their home environments, addressing their individual needs in a familiar setting. Key Responsibilities
  • Conduct thorough examinations and evaluations of patients to identify functional limitations and design tailored treatment plans.
  • Deliver patient care with adherence to customer service standards, ensuring respectful and effective communication with individuals of all ages and backgrounds.
  • Perform treatment interventions as outlined in patient plans, regularly monitoring progress and making necessary adjustments to maximize outcomes.
  • Document all patient interactions and service provisions accurately in both written and electronic formats, complying with relevant laws, rules, and regulatory standards.

Patient Population / Client Focus
