Job description

Working closely with community agencies, social workers and the Market-Based Population Health teams, at Privia Health, the Community Health Worker will provide assessment of patient needs, make community referrals, and follow up with patients who are identified as benefiting from additional support and intervention from their health care team.

Minimum Qualifications:

  • Education: High School Diploma
  • Licensure: Certified Community Health Worker (within 1 year of hire date) Company will provide training options
  • Experience: Minimum of 1 year of experience in social service or healthcare setting
  • Experience working with at-risk patients is highly desirable.
  • Skills: Strong communication, and interpersonal skills.
  • Ability to work in a fast-paced environment and manage multiple priorities.
  • Proficiency in electronic health record (EHR) systems.
  • Competencies: Empathy, critical thinking, and the ability to work collaboratively with diverse populations and interdisciplinary teams.

Interpersonal Skills & Attributes:
