Job description

Description:

This position is responsible for overseeing the daily operations of the Case Management Department and the Implementation of policies and procedures that will contribute to successful service delivery and positive client outcomes. The Senior Case Manager is also responsible for providing support and accountability to on-site Case Management Staff and interns and for collaborating with other departments within the organization to improve processes and carry out the Organizationโ€™s mission. The Senior Case Manager may also be required to intervene in crises, bridge the gaps in service delivery, and fill Case Management needs.

Responsibilities and Duties:

Program Development and Planning

  1. Collaborate with the Manager of Social Services and the Leadership team to develop and implement policies and procedures for the Case Management program.
  2. Identify areas for improvement and develop strategies to enhance program processes and outcomes.
  3. Evaluate program effectiveness through collection of data, assessment of processes, and obtaining client feedback
  4. Keep up to date on best practices and policy changes as it relates to Case Management

Team Leadership

  1. Provide guidance and support to Case Management staff and Interns in their job roles
  2. Arrange and conduct training and workshops to ensure that Case Management staff is kept up to date on changing trends policies and procedures that will affect service delivery
  3. Conduct regular team meetings, Interviews as needed, and performance evaluations and hold staff accountable to the Organizationโ€™s standards
  4. Encourage professional growth and provide resources for continued learning and development.

Case Management

  1. Oversees the day-to-day operations of the Case Management program
  2. Intervene in crises and emergencies and provide Case Management support to departments as needed
  3. May be required to provide transportation to clients or for client services

Collaboration and Coordination

  1. Identify and connect with service providers and community partners that may provide a benefit to the client population served
  2. Arrange tours and community meetings with other service providers to provide education to the Case Management Team
  3. Collaborate with other departments to refer and obtain needed services for clients
  4. Under the direction of the Care Services Director, coordinate with Universities/Colleges to provide field education to Social Work Students

Documentation

  1. Keep detailed internal notes for each client in a secure location. Maintain privacy and confidentiality of all notes taken.
  2. Review and transfer Intern notes to the Homeless Management Information System HMIS
  3. Enter service transactions in HMIS for each client interaction and service delivery
  4. Keep up to date on best practices and privacy guidelines related to documentation.

Supervision

  1. Assist in supervising Social Work Interns from local Universities/Colleges
  2. Assist in the interviewing and orientation of new social work Interns.
  3. Assist in conducting training with social work interns as a group and individually.
  4. Be an ongoing resource and coach for interns throughout their internship experience.

This is not a full description of job duties and responsibilities and additional duties may be assigned by the Director of Care Services and SALTโ€™s Leadership Team.

Qualifications:

Requirements

  • Must have a Bachelor in Social Work (BSW) or equivalent degree
  • Masters Preferred
  • Excellent verbal and written communication skills
  • Strong problem-solving and interpersonal skills
  • Organized accurate, thorough, detail-oriented, and able to monitor work for quality
  • Intermediate level with MS Office Suite and Google Docs

Preferences

  • Experience in working with the homeless population

Physical Requirements

  • Ability to lift to 50 pounds
  • Ability to stand, walk, sprint, run, and patrol for extended periods
  • Must hear and see well (either naturally or with correction)
  • Must be able to bend, stoop, squat, kneel, push, pull, and reach overhead
  • A considerable amount of time making repetitive motions
  • Work in a fast-paced environment with frequent interactions with customers
  • Ability to function with frequent temperature changes between indoors and out
  • Work in various weather conditions, including heat, cold, and rain
  • Stand and walk continuously for up to 8-10 hours during a shift while providing customer care
  • Considerable amount of time using a telephone and computer
  • Sit at a desk and use a computer for extended periods to perform data entry and

administrative tasks

  • Answer phone calls and communicate with colleagues and clients verbally throughout

the day
