Job description

Citizens Memorial Hospital (CMH) is recruiting a Transitional Care Management Registered Nurse to support patients post-discharge in their transition from an acute care setting to self-management of their condition at home for patient safety. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, therapies and more, CMH has its community covered!
About the area: Located in the heart of the Ozarks, our main campus in Bolivar has a hometown feel and affordable housing options of rentals or apartments. Springfield is only 40 minutes away with Battlefield Mall, Big Shots golf, Wonders of Wildlife, Urban Air and many other family attractions and restaurants to visit. Only an hour away is Branson, which includes many shows to see, Silver Dollar City and shopping!
About the department: Medical Home strives to improve patient outcomes by facilitating care provided within the Patient Centered Medical Home and providing support through transitions in care.
Responsibilities include: Patient education of their condition, strategies for successfully managing their health for improved outcomes, utilization of motivational interviewing to help initiate healthful behavior change, and coordinating linkage to the appropriate outpatient care modalities for ongoing care management as indicated.
A typical day includes:
  • Establishing direct relationships with patients by phone
  • Working with patients and care team to meet patient needs for expected recovery outpatient and safety assurance
  • Performing medication reconciliation and clinical assessment through verbal interview
  • Providing education and support to both patients and their family or caregivers
  • Identifying resource needs and referring as needed
  • Providing follow-up for patients as clinically indicated

Job Requirements:
  • Education: Completion of an accredited program (Registered Nurse)
  • Experience: At least one year of related clinical experience is required, preferably related to hospital nursing (e.g. ICU, Med-Surg, ED, psych)
  • Skills: organization, able to work in a fast-paced environment, multitasking, patient care, strong written and verbal communication.
  • Licensure/Certification: Active Missouri License as a Registered Nurse.
  • Physical: ability to work at a computer 90% of the day and stand for the remaining 10%
  • Experience and/or interest in behavioral health, palliative care, patient education, and/or disease management (e.g. DM, CHF, COPD) is applicable and beneficial for this patient support role.

The position is: Part Time, Days
Monday and Tuesday required, 8:00a.m- 4:30p.m, Additional hours flexible, no weekends, holidays, nights, or on-call.
