Job description

RN Patient Care Coordinator (Foregut) Clinic based role with EPIC outpatient REQUIRED Ambulatory experience REQUIRED Shift: 5X8 Days; 8:30AM - 5:00PM (hours may vary) On Call: No Contract Length: 13 Weeks Licenses/Certifications: BLS, BSN EPIC experience is a requirement The OHSU Digestive Health Center (DHC) is a multi-disciplinary center offering services in the ambulatory setting for specialty surgical services, gastroenterology, hepatology, bariatrics and nutrition. A key program within the DHC is the esophageal / foregut surgery section. This program is a close team that includes MD, APP, RN, MA, and schedulers that work together to provide excellent quality care for these complex patients. The patient experience with the program begins with their initial appointment and can include clinic visits, procedures, surgery, and follow-up care. The Patient Care Coordinator (RN) will provide care coordination, assessment and triage, medication management and clinical support to the foregut surgeons in the ambulatory / clinic setting. Foregut surgery is a specialized field where the team nurse gets to know many of the patients closely and plays a key role in patient education and support. The practice requires management of both benign and malignant expertise. The nurse will become familiar over time with the various treatment options and ongoing complex care. The nurse will also perform medical record review, follow developed protocols, determine appropriate patient scheduling, assist with monitoring diagnostic results and patient care progress/processes as requested by providers, and participate in quality reviews.
