The Role
We're hiring a full-time Medical Assistant to support our nurse practitioner in delivering high-quality clinical care and chronic care management (CCM) services to our senior patient population. This is an in-office role with a dual focus — managing and triaging all inbound calls from clients, families, and partners, and supporting our Nurse Practitioner with her administrative needs: faxing and tracking orders, coordinating referrals, communicating results, and documenting CCM activity, among other things.
What You'll Do
- Serve as the first point of contact for inbound calls from clients, families, and referral partners — triaging needs and routing appropriately
- Coordinate orders and manage the outflow of tasks generated from Nurse Practitioner home visits
- Track and follow up on outstanding orders, referrals, and specialist coordination to ensure nothing falls through the cracks
- Fax, retrieve, and file clinical documentation including orders, labs, and consult notes
- Communicate order results and relevant clinical updates to the NP in a timely manner
- Conduct monthly CCM check-in calls with patients, documenting status and flagging concerns
- Assist with care plan updates and CCM documentation under NP direction
- Support medication reconciliation, including reviewing current medication lists and flagging discrepancies
- Handle prior authorizations, scheduling, and other clinical support tasks as needed
- Maintain accurate and timely documentation across all systems
- Assist with general office duties including supply management, mail, and maintaining an organized clinical workspace
What We're Looking For
- Certified or Registered Medical Assistant (CMA or RMA preferred)
- Experience working with geriatric or chronically ill patients a plus
- Strong phone presence — you'll be the voice of RIISEN for clients and families daily
- Highly organized with strong follow-through; this role manages a lot of moving pieces
- Strong documentation habits and attention to detail
- Team player who takes initiative and communicates proactively
- EMR experience required; platform-specific training available
