The APC will be responsible for providing high-quality patient care, collaborating with physicians and other healthcare professionals, and ensuring that all clinical practices are in line with the latest medical standards. This specific role involves caring for our highest-risk patients, in collaboration with our Regional Medical Director.You WillPatient Care: Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic tests. Develop and implement treatment plans, including prescribing medications and therapies. Provide patient education and counseling on health maintenance, disease prevention, treatment options, and goals of care.Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive patient care. Participate in interdisciplinary team meetings and case conferences to discuss patient care and treatment plans.Quality Improvement: Monitor and evaluate patient outcomes to identify areas for improvement in care delivery. Participate in quality improvement initiatives and research projects to enhance clinical practices and patient outcomes.Documentation: Maintain accurate and detailed patient records, including medical histories, treatment plans, and progress notes.
