The Job Manages programs and leads clinicians providing primary care within integrated care delivery model to SCAN members. You Will Manages programs and clinicians providing primary care to SCAN members within the integrated care delivery model at SCAN. Manages the performance of clinical teams to achieve defined clinical care outcomes. Oversees clinicians in daily operations, ensuring adherence to ethics and quality standards in the delivery of patient care by Advanced Practice Providers (AAP). Selects, on-boards, trains and develops clinicians within the integrated care delivery model. Accomplishes staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching and counseling employees; initiating and enforcing systems, policies, and procedures. Provides clinical care to include performing exams, ordering diagnostic and screening tests, developing plans of care, accurately documenting patient medical history and care, and educating and consulting with patients and families. Collaborates with senior management on the resolution of operational and performance issues and the development of new plans and programs. Improves and integrates APP functions and systems across specialties and departments, including the development and implementation of APP Best Practice Models. Develops and monitors success metrics and adjusts programs as needed to improve efficiencies and effectiveness. Networks and builds relationships with internal and external key stakeholders to promote membership growth and quality outcomes for integrated care delivery products. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; and participating in professional societies. Maintains client confidence and protects operations by keeping information confidential. Contributes to team effort by identifying and achieving goals/targets as needed. Actively support the achievement of SCAN’s Vision and Goals. We seek Rebels who are curious about AI and its power to transform how we operate and serve our members. Supervises/Manages Others. Your Qualifications
  • Master’s degree from an accredited school of nursing.
  • Oregon RN, NP, and Furnishing licensure required.
  • Certification in area of specialty required.
  • 5+ years of recent NP experience required.
  • 3+ years supervisory experience preferred.
  • Demonstrated experience in working with both Medicaid and Medicare.
  • Strong strategic thinking skills to facilitate the development and delivery of new clinical products and services to seniors and the community.
  • Strong interpersonal skills, excellent written and oral communication skills; commitment to detail.
  • Demonstrated leadership and management skills.
