Key Responsibilities

Qualifications
  • MD or DO degree from an accredited medical institution.
  • Successful completion of an accredited residency program in Family Medicine.
Certifications
  • Board Certified in Family Medicine.
  • Eligible for (or currently possess) an Illinois medical license.
  • BLS and ACLS certifications are recommended.
Skills and Experience
