Responsibilities:
  • Completing the nursing process in the delivery of patient care, which includes assessment, planning, implementation, and evaluation of patient outcomes.
  • Total oversight of patient from admission to discharge.
  • Developing a plan of care based on findings.
  • Serving as patient's advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
  • Timely completion of all skilled nursing documentation.
  • Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs)
  • Multidisciplinary team approach (coordinating with all disciplines of patient care, including PT, OT, ST, MSW, HHA's, and office staff)

Requirements
  • Current Illinois State RN.
  • Bilingual (Russian/English)
  • CPR Certification
  • Previous experience in nursing or other health field experience
  • Familiarity with medical software and equipment
  • Ability to build rapport with patients
  • Strong problem-solving and critical-thinking skills
  • Ability to thrive in a fast-paced environment
  • WellSky EMR knowledge is preferred but not required.

Compensation and benefits:
  • Salary Range: $65-$85/per visit, commensurate with experience
