Patient Engagement Coordinator
Location: Rochester, NY
Reports to: Outreach and Education Manager

The Opportunity
This is not a traditional coordinator role.
As a Patient Engagement Coordinator, you'll be one of the first faces and voices of Town Square Health, meeting prospective patients in the community, following up with warmth and consistency, and making sure that the path from "I'm interested" to "I have an appointment" is as smooth as possible. You'll play a meaningful role in building our patient panel from the ground up, and your work will directly shape how people in the Rochester community experience us for the very first time.
The right person for this role brings genuine enthusiasm for connecting with people, a natural instinct for hospitality, and the follow-through to make sure no one falls through the cracks. Experience in healthcare is helpful, but a deep commitment to customer service and a passion for serving older adults will take you far here.
You'll have the opportunity to:
  • Be a trusted first point of contact for Medicare-eligible patients and their families
  • Represent Town Square Health in the Rochester community at outreach events and health fairs
  • Help shape what a welcoming, patient-centered experience looks like as we grow
  • Work alongside a collaborative team that is genuinely invested in doing healthcare differently

What You'll Do
You'll split your time between the clinic and the community by following up with prospective patients, supporting appointment scheduling, representing Town Square Health at outreach events, and greeting patients when they arrive. No two days will look exactly the same, and that's part of what makes this role exciting.
Patient Engagement & Recruitment
  • Monitor inbound prospective patient inquiries daily and ensure timely, warm follow-up by phone and email
  • Guide prospective patients through the process of scheduling their first appointment, answering questions and removing barriers along the way
  • Maintain accurate, up-to-date notes and tracking on the status of all prospective patient inquiries
  • Greet patients when they arrive and provide an exceptional, welcoming experience from the moment they walk in
  • Collaborate with the Outreach and Education Manager, Director of Growth Marketing, and RN Practice Manager on patient engagement and recruitment strategies

Community Outreach & Events
  • Attend community events as a representative of Town Square Health, engaging aging adults and caregivers to share information about our services and schedule appointments
  • Prepare outreach materials and logistics in advance of events in partnership with the Outreach and Education Manager and Director of Growth Marketing
  • Bring energy, professionalism, and genuine hospitality to every community interaction

Qualifications
  • High school diploma, GED, or equivalent
  • 1-3 years of experience in a customer service, hospitality, or community outreach role
  • Excellent communication skills and a natural ability to connect with people across backgrounds and ages
  • Strong task management and follow-through; comfortable managing multiple inquiries at once
  • Ability to work occasional evening and weekend hours for community events
  • Reliable transportation for community outreach events
  • Experience working in a healthcare setting is a plus, not a requirement
  • Experience with phone-based customer service is a plus
  • Mission-driven, collaborative mindset and a passion for improving access to high-quality primary care
