Description

All Metro Health Care, a Modivcare Personal Care Service, is looking for a Clinical Supervisor. You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides.

  • Pay: $86,880 to $89,000 based on experience 
  • Schedule: M-F 8:30AM-5:00PM 
  • Visiting Client Territory:  Buffalo area. Would need to be present in each specific area when the Manager of Clinical Services is unavailable for that area.

You will...

  • Performs on-site supervisory and teaching visits to evaluate caregivers
  • Provides clinical orientation to caregivers on assigned home care cases. 
  • Participates in the training of new nurses and in-services for field staff.
  • Performs initial nursing assessments
  • Counsels patient/caregiver in meeting nursing and related needs.
  • Assigns and coordinates services provided.
  • Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs. 
  • Prepares clinical progress notes ensuring any changes in the patient's condition is reported 
  • Monitors clinical records for complete and accurate documentation.
