Role Responsibilities:
  • Provide companionship and conversation to enhance the lives of our clients.
  • Assist with daily activities such as meal preparation, light housekeeping, and errands.
  • Provide physical assistance such as bathing, dressing, grooming, transferring, and ambulation.
  • Accompany clients to appointments or social outings, providing transportation as needed.
  • Monitor and report changes in the clients health, behavior, and needs.
  • Act as a member of the Care coordination team to communication with the RN Case Manager to update the plan of care.
Requirements:
  • A compassionate, patient, and caring nature.
  • Strong interpersonal and communication skills.
  • Experience providing care to clients in a hands-on environment (Home Health, Assisted Living, Group Home, Hospital Setting)
  • Reliable vehicle and a valid drivers license with Auto Insurance.
  • Negative TB test or Cleared Chest-Xray.
  • Availability to work 2 weekend days per month.
