Job description

Employment Status: Full time โ€“ Shift: Weekends Only (United States of America) โ€“ Facility: 390 S Main St, Rocky Mount.

The RN Case Manager provides case management for assigned patient populations, utilizing clinical expertise and communication skills to achieve optimal clinical and resource outcomes. The role promotes costโ€‘effective care by minimizing fragmentation, maximizing coordination, and facilitating patient/family movement through the health care organization. The RN Case Manager performs patient needs assessments upon admission and at regular intervals, facilitates referrals and provides linkages to health, wellness, and postโ€‘acute care resources across the continuum. The RN Case Manager promotes interdisciplinary collaboration and teamwork to progress the plan of care and discharge plan. The RN Case Manager promotes appropriate length of stay, resource management, and care transitions, complying with all federal and state regulations surrounding the discharge process.

Daily Tasks / Expectations
  • Participate in Unitโ€‘based IDR morning and afternoon huddles.
  • Coordinate referrals for DME, HH, Hospice.
  • Utilize predictive analytic software (e.g., JVION).
  • Complete faceโ€‘toโ€‘face patient assessments.
  • Communicate with assigned UR nurse and SW partner.
  • Reassess patients and document status of referrals and barriers.
  • Aid in the delivery of regulatory letters (IM, HINN).
  • Integrate InterQual information during unit huddles and throughout the day.
  • Provide medication assistance (RX Help, CMAP) and initiate medication investigations.
  • Communicate postโ€‘acute care needs during inmate transitions.
  • Assist in acuteโ€‘acute and transitions of care.
  • Maintain awareness and anticipate unitโ€‘based patient needs.
  • Provide handโ€‘off communication of unit needs during weekday/weekend transitions.
Requirements
  • Registered Nurse with a Bachelorโ€™s degree (or equivalent 5 years RN experience).
  • Current licensure in Virginia as a Registered Nurse.
  • AHA BLSโ€‘HCP certification within 6 months of hire.
  • Three years recent experience in a clinical health care setting with direct patient care management responsibilities.
  • Preferable supervisory or leadership experience.
