Job description

As a RN Case Manager Part-time, your voice to influence patient care is valued and empowered at every turn –whether through open, collaborative relationships with your direct manager or more formal opportunities through hospital councils and national nursing initiatives. You'll help shape decisions that elevate both patient outcomes and the future of nursing.

Job Summary and Qualifications

As a Case Manager at our facility you will be joining a team that hasa passion for creating positive patient interactions and a dedication to caring for the multi-faceted wellness of those we serve. In this role you will:

  • Be responsible for the day-to-day direction of social service patient care, including quality care of the patient from admission to discharge.
  • Evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.
  • Act as a liaison between the facility and resources external to the organization.
  • Implement timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care.
  • Provide crisis intervention and support.
  • Assist the Case Manager in facilitating patient movement across the continuum of care and identify and track barriers to patient throughput.
  • Establish and maintain effective cooperative working relationships with social service staff, nursing staff, medical staff, and other departments.

Qualifications:

  • Current licensure as a Registered Nurse in the State of Utah or RN compact license from participating compact states.
  • Associate Degree in Nursing required; BSN preferred.
  • 2+ years of experience in case management or  3+ years of experience in clinical nursing required.
  • Certification in case management or utilization review preferred.
  • Knowledge of Utilization Review, DRG review, and Discharge Planning preferred.
