Description

The Registered Nurse (RN) provides integrated, person-centered nursing care within a Certified Community Behavioral Health Clinic (CCBHC). This role supports individuals with mental health and substance use disorders through assessment, coordination of care, medication support, and collaboration with interdisciplinary teams.

As part of the CCBHC model, the RN plays a key role in delivering whole-person care, including behavioral health, physical health monitoring, and crisis response services.

Essential Functions:

  • Conduct comprehensive nursing assessments, including physical and behavioral health screenings
  • Monitor and document vital signs, medication adherence, and overall health status
  • Administer medications and support medication-assisted treatment (MAT) as applicable
  • Collaborate with providers (psychiatrists, PMHNPs, therapists, care coordinators) to support integrated treatment planning
  • Provide patient education on medications, diagnoses, and wellness strategies
  • Participate in crisis intervention and stabilization efforts, including coordination with mobile crisis teams
  • Support care coordination with hospitals, primary care providers, and community resources
  • Assist with intake, risk assessments, and ongoing clinical documentation
  • Ensure compliance with CCBHC standards, state regulations, and organizational policies
  • Demonstrating the ability to engage and develop rapport with individuals presenting with a variety of symptoms/presentations.
  • Providing services that are person-centered, culturally relevant, and are non-judgmental and welcoming to diverse populations.
  • Documenting all services provided within expected time frames, meets service definitions and regulatory standards, is person-centered and meets quality improvement goals.
  • Demonstrating excellent organizational skills and effective use of time management.
  • Participating in and completing all required training courses in an accurate and timely manner.
  • Participate in on-call nursing support.
  • Other: Completes other administrative support duties and special projects as developed and assigned.

CCBHC-Specific Expectations

  • Deliver services aligned with a whole-person, trauma-informed care model
  • Participate in care coordination across multiple systems (medical, behavioral, social services)
  • Contribute to reduced wait times and increased access to services
  • Provide care regardless of a client's ability to pay or insurance status
Qualifications

Education and Experience:

  • Active Registered Nurse (LPN/RN) license in the State of New Mexico
  • Experience in behavioral health, substance use treatment, or community health preferred
  • Knowledge of psychiatric medications and co-occurring disorders
  • Ability to work in a fast-paced, multidisciplinary environment
  • Strong communication and documentation skills

Preferred Experience:

  • Experience working in a CCBHC or integrated care setting
  • Familiarity with crisis intervention or mobile crisis services
  • Bilingual (English/Spanish) preferred

Knowledge and Skills:

  • Demonstrated success in the following areas:
  • Must demonstrate specific additional training or experience in psychological therapies, building a therapeutic alliance, dealing with challenging behavior and the administration of psychiatric medication.
  • Must be knowledgeable of psychotropic side effects and how evaluate for them (i.e. AIMS, observation).
