Job description

Patient Care Coordinator

FirstHealth of the Carolinas is a nationally recognized health care system located in central North Carolina. The Med/Neuro ICU is a recently renovated 20-bed unit which provides the highest-level acuity-adaptable care to medical, neurological, and surgical patients admitted to FirstHealth. MNU has been recognized for being CAUTI free for 2 years as well as being a Five-Star Recipient from Healthgrades for the treatment of Stroke and Respiratory Failure. We seek to develop and maintain a highly educated registered nursing staff to meet the present and future demands of the nursing profession.

The primary objective of the Patient Care Coordinator is to proactively manage patient flow while assuring effective, quality care. This is a highly visible position within the organization that requires exceptional communication and organizational skills. In partnership with leadership, the Patient Care Coordinator supports the assigned patient care area(s) and facilitates patient flow and a systematic flow and patient care. Duties include leading shift huddles, completing patient assignments, supporting daily operations, assuring team members meals, first-line resolution of patient/team members/provider complaints, elevating appropriate items to the Nurse Manager/Director of Nursing. Additionally, the Patient Care Coordinators support organizational quality initiatives and throughput goals as well as completion of required daily reports and checklists.

  • Problem solving, flexibility, interpersonal communication skills and the ability to remain calm under high levels of stress
  • Ability to comply with all safety and health standards, and all rules, regulations, and orders which are applicable to the individual's own position, actions, and conduct.
  • Demonstrates clear and concise verbal and written communication skills.
  • Able to perform multiple tasks at once in an organized manner.
  • Collaborates with nursing leadership to ensure resources are available to perform patient care.
  • A model for service excellence with outstanding communication and feedback skills
  • Ability to effectively function in all departmental assignments
Qualifications

Required:

  • Current North Carolina RN licensure (or compact state)
  • Minimum two (2) years patient care experience

Preferred:

  • Bachelor of Nursing
  • Specialty Certification
  • Experience working as a charge nurse in an acute care setting
