Position Summary
The primary purpose of your job position is to foster independence and promote socialization for each resident of the Enriched Housing Program. Assist residents with activities of daily living and housekeeping duties as indicated by the Resident's Individual Service Plan.
Essential Job Functions:
  • Assist residents with Activities of Daily Living at all levels including Assisted, Enhanced, and Special Needs Levels of Care.
  • Prepare and maintain documentation/records of services performed.
  • Perform light housekeeping duties i.e. trash removal, laundry, bed making.
  • Consult/report to nurse supervisor Resident's needs and any changes in Resident's status.
  • Attend and participate in any mandatory meetings, in-services and additional training.
  • Serve between meal and bed time snacks as needed.
  • Assist the LPN Supervisor, Shift Supervisor, Case Manager/Nurse Manager, and/or Assistant Executive Director in planning, developing, organizing, implementing, and evaluating resident care services, in accordance with the current rules, regulations and guidelines that govern the Community.
  • Participates in and receives the shift report upon reporting for duty.
  • Weighs and measures residents as instructed.
  • Assists with dining service during mealtime.
  • Assists with Life Enrichment Activities as directed

Job Requirements:
  • Requires ability to relate and be respectful to a wide variety of people and provide superior customer service to all including Residents, co-workers, vendors, family members and visitors.
  • Must wear employee badge at all times while on-the-job.
  • Must report to work on time, adhere to attendance policy and punch in and out for all meal breaks.
  • Must be able to maintain a high level of confidentiality regarding information about Resident, co-workers, vendors, family members and visitors.
  • Meets with other Nurse Manager/Case Manager/Assistant Executive Director on a regularly scheduled basis, to assist in identifying and correcting problem areas, and the improvement of services.
  • Reports all complaints and grievances made by the resident to the LPN Supervisor, and/or Shift Supervisor.
  • Reports occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to the nurse supervisor.
  • Responds to resident calls promptly and calls from the Personal Emergency Response System (PERS).
  • Cleans, disinfects, and returns all resident care equipment to its designated storage area after each use.
  • Wears and uses safety equipment and supplies as required.
  • Immediately notifies the LPN Supervisor, Shift Supervisor, Nurse Manager, Case Manager and/or Assistant Executive Director of any resident eloping/missing from the community.
  • Operates all equipment in a safe manner and reports defective equipment to supervisor.
  • Ensures that all Resident's rights are upheld.
  • Other duties as deemed necessary and appropriate, or as may be directed by your supervisor.
  • Must have reliable transportation.

Expectations of the Position:
  • Dependable, report to work on-time for every schedule shift (ready to work at beginning - start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO), which includes personal and vacation.
  • Willing and able to work overtime and weekends as required by the position and/or approved by management.
  • Have reliable transportation to and from work.
  • Must be clean and neat in appearance and have a "friendly," hospitality-oriented, and service-oriented demeanor.
  • Abide by work-related/job procedures and organization policies; (refer to the Employee Handbook).

Qualifications of the Position
  • A combination of experience, education, and/or training which substantially demonstrates the following knowledge and skills:
  • Must be at least 18 years old at time of hire.
  • Education: High School graduate or equivalent
  • Experience: At least 1 year experience working in assisted living facility assisting residents with daily living activities.
  • Skills:
  • Licenses/Certifications: HHA or PCA certification preferred
