Key Responsibilities
  • Document and complete critical incident reporting forms within 24 hours of occurrence to ensure compliance and safety.
  • Provide after-hours phone support on an alternating basis to facilitate care coordination for clients in need.
  • Engage proactively with participants to employ preventative strategies that minimize the need for crisis services, enhancing client safety and emotional well-being.
  • Develop and maintain detailed crisis alerts in coordination with county crisis services, ensuring rapid responses to urgent client needs.
  • Collaborate closely with clients through direct counseling sessions, establishing therapeutic relationships focused on trust and rapport.
  • Utilize clinical expertise to assess client needs and tailor interventions that facilitate recovery and resilience.
  • Participate in team meetings and training sessions to share knowledge and strategies aimed at improving service delivery and client outcomes.
