Key Responsibilities
  • Conduct thorough psychosocial assessments of clients, which may include individuals, adolescents, families, and groups, enabling a comprehensive understanding of their needs.
  • Create and develop appropriate treatment plans, utilizing graduate-level clinical social work/counseling interventions tailored to the unique situations of clients.
  • Engage in routine communication with clinical staff regarding assessment outcomes, ongoing treatment strategies, and interdisciplinary care plans during collaborative meetings.
  • Provide case management services, ensuring that patients receive comprehensive care by collaborating effectively with hospital and community professionals.
