• Conduct thorough psychosocial assessments utilizing interviews, observations, and the review of medical records to identify emotional and social factors affecting patient recovery.
• Deliver individual and group counseling, providing emotional support to patients and families while addressing adjustment issues and fostering resilience.
• Collaborate with a multidisciplinary rehabilitation team to create integrated, individualized treatment plans focused on psychosocial well-being as part of the patient’s overall care.
• Guide patients in successfully accessing community resources, including vocational rehabilitation services, housing assistance, and support groups to enhance their recovery process.
• Educate patients and families on available post-rehabilitation services and community resources, ensuring seamless transitions and sustained support after discharge.
• Engage in discharge planning activities, facilitating smooth transitions to home or alternative care settings while ensuring continuity of care and service delivery.

Our patient population includes individuals with a range of rehabilitation needs stemming from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. The salary for this position ranges from USD 75,000 to 100,000 annually, based on experience and qualifications.
