You’ll engage with colleagues through virtual meetings and utilize advanced electronic health records (EHR) systems to maintain up-to-date patient information. In your role as a Licensed Independent Clinical Social Worker, you will have the following key responsibilities:

• Conduct thorough assessments of patients’ social and emotional needs and collaborate with healthcare professionals to develop tailored treatment plans.
• Effectively communicate patient needs to the interdisciplinary treatment team, facilitating cohesive care strategies aimed at achieving optimal discharge goals.
• Design and implement comprehensive discharge plans in coordination with healthcare providers, ensuring seamless transitions and continuity of care for patients.
• Engage in ongoing professional development through skills training, workshops, and educational opportunities to enhance your clinical competencies.
• Document all patient interactions and updates accurately in our electronic health record systems, adhering to regulatory standards and organizational protocols.
• Undertake special projects and other duties as assigned, contributing to the overall success and improvement of our services.

The primary focus of this role will be patients in hospice care, many of whom face complex emotional and psychosocial challenges. You will work closely with individuals navigating end-of-life issues, providing compassionate care to improve their quality of life while offering support to their families. Your ability to empathetically engage with patients and their loved ones will be essential in fostering a trusting environment that promotes healing and emotional well-being.

• A Master’s degree in Social Work from an accredited institution.
• A valid Licensed Independent Clinical Social Worker (LICSW) license in the state of Washington.
• Strong interpersonal skills to effectively engage with patients, families, and a diverse treatment team.
• Current Basic Life Support (BLS) certification, ensuring readiness in emergency situations.

Candidates with a minimum of three years of clinical experience in social work, particularly in hospice or palliative care settings, will be preferred. Experience in collaborative interdisciplinary care is also advantageous to enhance patient advocacy and interdisciplinary communication.
