The Marriage and Family Wellness Hub is seeking a dedicated and compassionate Full-Time Licensed Clinical Social Worker (LCSW) to join our dynamic team, providing essential support to clients within a hospice care environment. This key position involves delivering high-quality psychosocial assessments, counseling, and therapeutic support to individuals and families facing end-of-life challenges. Key Responsibilities
  • Deliver individual, family, and group counseling tailored to the unique needs of hospice patients and their families.
  • Conduct thorough interviews to collect and interpret essential social, personal, environmental, and health information to inform care plans.
  • Utilize established methodologies to evaluate and address psychological and social issues within the multi-disciplinary framework of hospice care.
  • Create and implement structured programs and activities that support clients’ emotional and psychosocial needs throughout their hospice journey.
  • Provide immediate crisis intervention and facilitate access to community resources, ensuring that patients receive timely support.
  • Establish trusting relationships with clients based on respect, acceptance, and empathy, which are crucial for effective therapy.
  • Regularly assess the effectiveness of treatment, modifying approaches as necessary to enhance patient outcomes.
  • Maintain accurate and thorough documentation of patient progress, treatment plans, and case notes in compliance with healthcare regulations.
  • Manage a personalized caseload, ensuring up-to-date records reflect the continuum of care offered to each client.
  • Perform additional duties as assigned, which may include participation in team meetings and training programs.
Patient Population / Client Focus

The primary focus of this role will be on patients in hospice care, dealing with a range of complex emotional issues associated with terminal illness. This includes providing support for grief, loss, anxiety, depression, relationship issues, and family dynamics during this critical time. You will be instrumental in ensuring that both patients and their families receive comprehensive psychosocial support to foster peace and acceptance.

Qualifications
  • A Master’s degree in Social Work, Counseling, Psychology, or a closely related field is required.
  • Must hold an active License as a Clinical Social Worker (LCSW) in the state of Florida.
  • A minimum of 4 years of experience in a social work or mental health setting is necessary, with a strong preference for experience in hospice or palliative care.
Preferred Experience
  • Prior experience in assessing and treating a range of behavioral health conditions, including depression, anxiety disorders, substance use, and grief counseling.
