Key Responsibilities
  • Conduct ongoing assessments to determine patient needs and create customized treatment plans, ensuring all assessments comply with legal and ethical standards.
  • Maintain clear and open communication with supervisors, direct care staff, family members, and guardians regarding each client's progress and treatment outcomes.
  • Develop and provide a tentative schedule of appointments for clients, home visits, and observations to monitor treatment effectiveness.
  • Coordinate and execute home visits to gather essential information through observations, chart reviews, interviews, and data analysis to support treatment plans.
  • Ensure the implementation of program plans is effective and aligned with the established therapeutic goals.
  • Actively participate in Interdisciplinary Team Meetings and contribute to discussions with insights gathered from your clinical interactions.
  • Timely completion of monthly and quarterly progress notes to reflect client progress and treatment adjustments.
  • Conduct training sessions to empower staff and team members to execute and document treatment plans competently.
  • Provide continuous follow-up through interviews and check-ins, ensuring supportive measures are in place for ongoing client care.
