Overview:
Sentara Health Plans is hiring a Social Worker/ Masters in Social Work (MSW) for Maternal Health/ Perinatal for Central Richmond (Henrico, Hanover, Colonial Heights, VA and the surrounding areas)!
The position requires both telephonic and face-to-face assessments and remote telephonic assessments for the low-risk and high-risk pregnant population (Maternal Health / Perinatal / OB-GYN /Labor and Delivery / Mother-Baby / Postpartum) for Central Richmond (Henrico, Hanover, Colonial Heights, VA and the surrounding areas)!
Status: Full-time, permanent position (40 hours)
Work hours: 8am to 5pm EST, M-F
Location: Candidates must reside in Central Richmond (Henrico, Hanover, Colonial Heights, VA and the surrounding areas)
Job responsibilities:
Social Worker works in a collaborative role with all members of the health care team and community service agencies to coordinate care for Sentara Health Plan members to improve health outcomes. Assesses for barriers to compliance with the medical care plan and implements appropriate interventions to promote improved health and compliance with recommended therapy. Acts as a resource to assist members in need of community-based program assistance and empowers the member with the skills to navigate the process for obtaining appropriate medical care and community assistance.
Education:
  • MLD - Master's Level Degree Social Work REQUIRED

Certification/Licensure:
  • Licensed Clinical Social Worker (LCSW) preferred

Experience:
  • 3 years of experience as a Social Worker REQUIRED
  • Social Work in physician office setting, CSB, home health, long term care, acute care or managed care setting experience REQUIRED
  • Experience working with low and high risk pregnant population/Maternity/OB/L&D/Mother Baby/Postpartum experience preferred
