This role is designed for those who thrive in a remote work environment. The position requires effective collaboration with a diverse range of professionals, including physicians, nurses, and other healthcare providers within the hospice center. You will leverage technology to facilitate communication and team coordination while remaining deeply connected to the clients and families you serve.

As a Senior Clinical Social Worker, your key responsibilities will include:

• Collaborating with multidisciplinary teams to assess client needs and develop comprehensive, individualized care plans that reflect both psychological and practical support for clients facing end-of-life challenges.
• Conducting thorough clinical assessments to understand clients’ emotional, social, and spiritual needs, enabling the creation of actionable treatment plans tailored to each individual.
• Providing individual and family counseling, offering emotional support and coping strategies to help clients and their loved ones through difficult times.
• Facilitating discharge planning and coordinating resources to ensure seamless transitions to other levels of care when necessary.
• Staying informed on current evidence-based practices and innovative approaches in the mental health field to enhance client care and outcomes.

Your role will primarily involve working with a vulnerable population—clients who are in hospice care and may be facing terminal illnesses. This position is designed for individuals who require not just clinical support, but also emotional and spiritual care. You will engage with clients from diverse backgrounds and their families, helping them navigate their circumstances with dignity, empathy, and respect.

The ideal candidate must have the following qualifications:

• A Master’s degree in Social Work (MSW) from an accredited program.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of New Jersey.
• Proven expertise in clinical evaluations, care planning, and psychosocial intervention methodologies.
• Strong communication and interpersonal skills, with an ability to work collaboratively within a multidisciplinary team.

A minimum of two years of clinical experience in a healthcare or mental health setting, particularly in hospice or palliative care environments, is highly desired. Familiarity with electronic health record (EHR) systems and an understanding of the regulations affecting social work in healthcare are also advantageous.

This position offers a competitive salary range between $90,000 and $110,000 annually, complemented by a comprehensive benefits package that includes:
