You will work from your home office while coordinating care through a user-friendly application designed to streamline case management and communication. • Conduct thorough psycho-social assessments and identify patients' needs to establish effective treatment plans.
• Diagnose and treat various mental, emotional, and behavioral disorders, adjusting care plans as necessary based on patient progress.
• Collaborate with healthcare providers, community resources, and patients' families to ensure comprehensive care throughout the treatment process.
• Provide individual and family therapy, crisis intervention, and supportive counseling sessions to facilitate recovery.
• Refer patients to additional services, including community programs, support groups, and additional healthcare providers, to enhance their support network.
• Maintain accurate documentation and comply with all federal, state, and local regulations, as well as organizational policies.
• Participate in the creation of discharge plans that prioritize continuity of care and post-treatment support.
• Act as an advocate for patients, ensuring their voices are heard within multidisciplinary teams and during care discussions.
• Identify potential barriers to patient care, such as financial or logistical issues, collaborating with other professionals to find solutions.

You will work with a diverse clientele, encompassing individuals facing a variety of mental health challenges. This may include anxiety disorders, mood disorders, trauma-related issues, and more complex psychological concerns. Engaging with clients from various backgrounds, your role will require cultural competence and sensitivity, ensuring personalized care tailored to each individual's unique situation.

• A Bachelor's Degree in Social Work or a related field from an accredited institution.
• Current state licensure as an LCSW, LMFT, LMHC, or Psychologist.
